We are in-network with the following Insurance for Acupuncture:

Regence Blue Cross Blue Shield & Providence Health Plan.

WE ACCEPT HSA & FSA Cards: Please confirm with your HSA/FSA carrier if acupuncture is a covered services and if any referrals are required.

IF YOU HAVE OUT OF NETWORK COVERAGE with your Health Insurance Plan, we can supply you with a Superbill (medical coded receipt) that you may supply to your insurance for your reimbursement. Payment for these services are due at Time of Service.

Please note that we are Not in-network with Medicaid / OHP Plans.

At this time Medicare does not approve Acupuncture performed by an Acupuncturist, therefore we do not accept Medicare.

Your Insurance information provided by your insurance company is not a guarantee of payment.

We will do our best with billing. Please note that you are responsible for co-pays / co-insurance or deductible at your time of service, including any amount that your insurance does not reimburse Blue Horizon Acupuncture.

Not all conditions and services are covered by insurance. If your insurance company does not cover services for your visits for any reason, you are responsible for payment.

Please contact your insurance company to verify what your health benefits and deductible are for acupuncture services BEFORE your first visit. We can check your benefits prior to your first visit as a courtesy and confirmation of coverage, with a week in advance of your appointment.
